Terms of Service

1. Agreement to Terms

By using the Signature Vitality website (signaturevitality.net) or scheduling or receiving any service from Signature Vitality, you agree to these Terms of Service. If you do not agree, please do not use our services.

2. Services

Signature Vitality provides IV therapy, wellness plans, NAD+ therapy, Vitality Booth sessions, aesthetic services, and related wellness services administered by licensed Registered Nurses in Pittsburgh, PA. Our services are intended for wellness support and are not a substitute for medical diagnosis, treatment, or emergency care.

3. Health Eligibility & Informed Consent

You must be 18 years of age or older to receive services. Before receiving any service, you will complete a health intake and consent form. You agree to disclose all relevant medical history, current medications, and health conditions accurately. Certain medical conditions may disqualify you from receiving some or all services. Your nurse will advise you. Providing false or incomplete health information releases Signature Vitality from liability for any resulting adverse effects.

4. Not a Substitute for Medical Care

Our services are administered by Registered Nurses and are intended for wellness support. They are not intended to diagnose, treat, cure, or prevent any disease. Do not use our services as a substitute for advice from your physician or other qualified health provider. If you are experiencing a medical emergency, call 911 immediately.

5. Bookings, Cancellations & Refunds

Bookings: Appointments may be scheduled online or by phone. A booking is confirmed once you receive a confirmation message.

Cancellations: We request at least 24 hours' notice to cancel or reschedule an appointment. Late cancellations or no-shows may incur a fee at our discretion.

Wellness Plans: Sessions within a wellness plan never expire and may be shared with immediate family members. Wellness plan purchases are non-refundable once services have been rendered. Unused sessions may be transferred at our discretion.

6. Payment

Payment is due at the time of service. We accept major credit cards and other payment methods displayed at checkout. Prices are subject to change; the price displayed at booking confirmation is the price you will be charged.

7. Stored Payment Cards & Recurring Charges

Card on file. If you choose to save a payment card on file, you authorize Signature Vitality to securely store a tokenized reference to that card with our payment processor (North). We do not store full card numbers on our systems. Tokenization meets PCI DSS standards.

Visit charges. You authorize Signature Vitality to charge your card on file for any IV therapy, wellness plan session, booster, aesthetic service, or other product or service you receive, in the amounts displayed at booking or checkout. You also authorize charges for any no-show or late-cancellation fees consistent with §5.

Recurring membership and wellness-plan billing. If you enroll in a membership, wellness plan, or any program with recurring charges, you authorize Signature Vitality to charge your card on file on the frequency and in the amount disclosed at the time of enrollment (for example, monthly membership dues or scheduled wellness-plan installments). The amount of each recurring charge will not exceed the amount you authorized at enrollment unless we obtain your separate consent for a change.

Revoking authorization. You may remove a card on file or revoke this authorization at any time from the "Manage cards" section of your account, or by contacting us at SignatureVitality1@gmail.com or 412-440-8702. Removing a card does not cancel any active membership or wellness plan. You remain responsible for any outstanding balance and for providing an alternate payment method going forward.

Card-network rules. Stored credentials are subject to applicable card-network rules for merchant-initiated transactions. Receipts for each charge will be sent to the email on file.

8. Limitation of Liability

To the maximum extent permitted by law, Signature Vitality, its owners, staff, and nurses shall not be liable for any indirect, incidental, special, or consequential damages arising out of or in connection with your use of our services or website. Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim.

9. Website Use

You agree to use our website for lawful purposes only. You may not scrape, copy, distribute, or reproduce any content from our website without written permission. We reserve the right to refuse service or access to our website to anyone for any reason.

10. Intellectual Property

All content on this website (including text, images, logos, and wellness plan content) is the property of Signature Vitality and protected by applicable copyright and trademark laws. You may not reproduce or distribute any content without prior written consent.

11. Governing Law

These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles. Any disputes shall be resolved in the courts of Allegheny County, Pennsylvania.

12. Changes to These Terms

We may update these Terms from time to time. The current version will always be posted here. Continued use of our services after changes constitutes acceptance of the updated terms.
